Thanks for the pointer. For those specific questions I've added a further checkbox, stating "I'm using multiple versions" or similar.

single answers to most of the questions. E.g., Q1: I'm currently
developing some ADF apps, testing others, and maintaining still others in
production. Q2: Mostly 10.1.3, since a lot of my clients are too deeply
invested in Portal to change their AS, but 11g when I can. I occasionally
even fire up 9.0.4 to look at old apps, some of which I'm in the process
of updating to 10.1.3 (for a client who can't lose Portal yet). And so on.

Any chance of converting a few of those radio buttons to checkboxes?
